I am a contracted provider with Horizon Blue Cross/Blue Shield and other BCBS plans. Since there are innumerable variations within these plans, contact my office with any questions.

If you have a different insurance plan that offers benefits for treatment by out-of-network practitioners, I may be able to accept assignment of your out-of-network benefits; you would be responsible for your deductible and co-payment. Call for details.
